Fill out one form per participant by printing your details in capitals and return by Fax to ZITA CONGRESS & TRAVEL (Fax: +30 210 6642116, Attn.: Ms Natalia Tsakmakidou) or by e-mail to

Note: If you wish the conference to make arrangements for your accommodation please indicate your hotel preference and arrival /departure dates in section C of the Registration Form.

* Proof of student status (student ID or letter from academic supervisor) must accompany this form

Cancellation and refund policy

• All changes and cancellations can only be made in writing.
• All cancellations involve a € 25 handling fee.

In case of cancellation, the following policy applies:

• Registration fees and other events (conference dinner, cruise) Cancellations before 24/04/2009 will be fully refunded. For cancellations after 24/04/2009 no refunds will be made
• For hotel accommodation: Cancellations before 10/04/2009 will be fully refunded. For cancellations after 10/04/2009, a one-night deposit will be charged.

• Access to exhibition and poster Areas
• Conference bag, contents and abstract book
• Welcome reception
• Coffee and refreshments during the coffee-breaks of all conference days
• Buffet lunches during the lunch-breaks of all the conference days
• Guided tour to Acropolis area
  Please note: Conference dinner is not included in the registration fee.
